Care Ministry

Benevolence form: All requests for assistance must be made by completing a “Request for Assistance” form. Contact  at 317.203.7950, to request a form, pick up a form in the office or fill it out online.

Prayer request: Do you have a need that you'd like the Kingsway staff, elders and/or prayer chain to pray for? If so, submit this online form.

Stephen Ministry referral: Is someone you know in need of care? If so, please print, complete and return this form to Paul Smith's mailbox in the church office.

Communications

Promotion planner: Would you like to promote an event or some important news, complete this form online or print and return to the Communications mailbox in the church office. (This form needs to be received at least two weeks in advance.)

Facilities

Special set-up request form: Need a special room arrangement for your event? Complete this form and return it to Bobbette Sturgeon's mailbox in the church office two weeks before your event.

Involvement

Spiritual Gifts assessment: Have you ever wondered what your spiritual gifts are? Fill out this form and we'll send you a summary of your results.

Kids

Medical release form: We ask that all kids involved with Kingsway's Kids Ministry have a Medical Release Form on file. We use these forms in the event of a medical emergency during a youth event or trip. Print and return to Caitlyn Smelser's mailbox in the church office.

New volunteer application: We ask all volunteers who work with our kids to print and return a background check and family ministry application to Caitlyn Smelser's mailbox in the church office.

Room reservation form: Need to reserve a room at the church for your next Life Group event? Click here to fill out a room reservation form online.
